Arrival Time and Appointment Documents:
Please be sure to arrive 20 minutes before your scheduled appointment if you are a new patient and 15 minutes if you are an existing patient. This lead time is required to finish your registration and/or update your medical records (click here to download patient forms). Completed forms will accelerate your check-in process and save you time.
Please bring your insurance cards with you. We are required to verify your insurance eligibility at each visit. To do this we need to make a copy of your insurance cards at each visit. This makes it easier for us to bill your insurance for you.
Please bring a list of your current medications (including over the counter medications) to EVERY VISIT. Up to date list of your medications will enable us to provide the best possible care. The doctor will need to ensure that any new prescriptions do not counteract or conflict with your current medications.
For the most efficient and the fastest way of refilling your medication, please contact your pharmacy directly. Your pharmacy will contact us for an authorization. Your medications will be reviewed and refilled whenever appropriate. Please allow at least 48 hours for prescription refills.
At NorCal Medical Group our first priority is to our patients. We strive to provide service to our patients whenever they might need care. By scheduling and missing appointments, this most certainly results in the loss of an opportunity to another patient who might be in desperate need of an appointment. To minimize the occurrence of missed appointments and to compensate for the time of our staff, a $30.00 administrative fee will be charged for each missed appointment that is not canceled 48 hours prior to the scheduled appointment time.
All co-pays, fees and outstanding balances must be paid in full prior to seeing Dr. Kujok.

Emergencies do occur and we ask that you not hesitate to contact us with urgent problems. After hours, our phone is covered by our answering service. Dr. Kujok is always “On Call” for emergency care (24 hrs/day, 365 days per year). The service will relay urgent questions regarding your health care to Dr. Kujok.
